Perceived Success in Upper-Extremity Vascularized Composite Allotransplantation: A Qualitative Study

چکیده

Purpose We performed a qualitative study to understand psychosocial factors associated with perceived success of upper-extremity vascularized composite allotransplantation (VCA). interviewed transplant recipients and their primary caregivers. Methods recruited 4 VCA caregivers for 3 them. conducted semistructured face-to-face interviews using guide that explored participants’ transplantation experiences. Topics included comparison pretransplant posttransplant expectations, reflections on contributing the experience, rehabilitation functioning. thematic analysis produced list themes, subthemes, proposed hypotheses explaining how themes related study’s guiding questions. Results Participants described several as including developing realistic expectations about function lifelong immunosuppression, support from one’s community particularly caregiver, framing experience in positive light. Social, aesthetic, other values unique hands, opposed prosthetics, motivated undergo despite its inherent risk uncertainties. Conclusions Despite challenges, undergoing was viewed worthwhile regain benefits hands. met challenges process through setting strong social support, perspective. Clinical relevance Findings this work may help clinicians prospective patients prepare set appropriate VCA.

Upper Extremity Composite Tissue Allotransplantation Imaging

OBJECTIVE Upper extremity (UE) transplantation is the most commonly performed composite tissue allotransplantation worldwide. However, there is a lack of imaging standards for pre- and posttransplant evaluation. This study highlights the protocols and findings of UE allotransplantation toward standardization and implementation for clinical trials. METHODS Multimodality imaging protocols for a...

Facial Vascularized Composite Tissue Allotransplantation

Reconstruction of severe composite defects of the face is challenging for reconstructive surgeons. Despite several reconstructive techniques, the functional and aesthetic results of these patients are not acceptable. As a major organ, face, has an utmost importance in daily life. The specific characteristics of face require not a reconstruction, but a replace with same tissue. With the evolutio...

Split Rejection in Vascularized Composite Allotransplantation

INTRODUCTION Graft monitoring following vascularized composite allotransplantation (VCA) relies primarily on serial skin biopsy. However, given that VCA comprised multiple tissue types, skin biopsy may not accurately reflect rejection in other transplanted tissue. METHODS A review of the literature on episodes of both acute and chronic rejection following VCA was completed. Special attention ...

Mouse Models of Experimental Vascularized Composite Allotransplantation

The clinical practice of vascularized composite allotransplantation (VCA) has been limited by the potential side effects of chronic immunosuppression. Studies using rodent models have been useful for dissecting mechanisms underlying immunological events induced by VCA and for developing protocols such as mixed chimerism for tolerance induction. Mouse models of VCA have great advantages over rat...

Current status of vascularized composite tissue allotransplantation

Vascularized composite tissue allotransplantation (VCA) offers treatment options of complex functional deficiencies that cannot be repaired with conventional reconstructive methods. VCAs consist of blocks of functional units comprising different tissue types such as skin, bone, muscle, nerves, blood vessels, tendons, ligaments and others, and are thus substantially different from the compositio...
